The 1095-C, Employer-Provided Health Insurance Offer and Coverage form contains detailed information about the health care coverage provided by your employer(s). If you were enrolled in health care coverage from your employer, worked an average of 30 or more hours per week during an annual measurement period, or were considered a full-time employee at any point in 2015, then you should receive a 1095-C.

My Online Paystub and W-2

Associates may access their online paystub and W-2 at any time using the Doculivery Associate self-service system.

Former Associates have access to the Doculivery Associate self-service system for seven years after leaving Family Dollar.

Online Paystub

Associates may sign up to receive an e-mail and/or text message notification when their online paystub is available to view. Past paystubs may be viewed through Sept. 2009.

Online W-2

Current and former Associates may opt out of receiving a paper W-2 and receive their W-2 online only. Online W-2 delivery is a secure and faster method of delivery. Current and former Associates may also sign up to receive an e-mail and/or text message notification when their online W-2 is available to view. Previous years’ W-2s may be viewed going back seven years.
